Adding Multiple Input Boxes on a new Page

First time user of WordPress and I really don’t have much experience designing webpages, but here’s my question: How do I add input boxes on a new webpage and then email all the information submitted to an email address?

My goal is to have 9-10 input boxes so the person viewing the webpage can type in the requested info, then hit submit and have all the info they typed in sent to an e-mail address?

Please be as thorough and specific as possible if you can help me as I really don’t know what I’m doing. Thanks
