Unfortunately there is currently no option to add extra fields to the appointment form. We’ll note this down as a feature suggestion and look into the possibility of adding custom field functionality in a future update. Thank you.

I need to add a comment field to the form after the users information. They will need at describe their issue. How would I go about doing this?
I may also be interested in working on development of this plugin.
