Adding photos to comments

I would like to use a comment form in a new site that requires the user to complete a few lines of specific metadata with their comment and would also allow for the inclusion of a photo. Does anyone know either the specific code necessary, or an up-to-date plug-in, that would allow me to set up this style of comment form, most importantly the addition of photos? Thanks.
