Adding Text Box for End-User Entry

Thank you for such a great plugin! It works fantastic!That being said, I think I may have asked you this before, but I cannot find history of this anywhere! 🙁
I am the leader of a gaming clan (http://www.ak907.net) and we require our members to donate so we can afford to pay for Battlefield 3 servers.
Priority 1
What would be a nice thing to have, is the option for the end-user to enter text into a text box, so they can leave their in-game name and some details (if they so choose).I think that is relatively easy to do, and I’ve done it before by modifying the code in one of the files, but like I said I cannot find your notes from the last time we talked about this and I think some updates to your plugin have overwritten the custom code I added.

On top of the option to add a text box, it would also be convenient for them to be required to enter something in that box before they can "submit".I know the first one is possible, and I would very much appreciate your guidance in setting this back up.
The second one I’m not too sure about, but if it’s possible I would love to see these as options in future releases.
