Adding to Media Library Uploading

I know adding files to the media library is easy. For some reason my media library let me add one file and now everytime I try and upload another file to the media library, it shows it being uploaded, says upload 100% then the file is no where to be found. This is probably an easy fix. What am I doing wrong? Am I looking in the wrong place? Thanks.
