This looks like it is related to some specific CSS of the site. Most likely it’s because of the theme but it might also be related to some CSS added by other plugin or a custom CSS used on site.
I can’t replicate the issue on my test setup with a standard Twenty Twenty theme so maybe I could actually see it “live” on your site?
It would be great if you could share a link to the page where we could see and test this particular form?

I’ve visited the page but I’m afraid I don’t see any “address menu” or any form there. This is all I can see on the page:

Did you remove the form or is it hidden for logged-in access? Could you please make it publicly visible so I could actually check it and examine the CSS?

Yes my bad it was hidden i fixed that, you can check it now.

Could you please add the following CSS rule at the very bottom of Appearance -> Customize -> Additional CSS?

}
