Address Not Saving

I have used this plugin on my friend’s website, and it worked perfectly. It was very easy to set up and get going. Now installed on a different website, I tried creating a Placemark. The “address” field will not save no matter what I have tried. I’ve tried zip codes, city and state, even geocode. Each time I hit update, the field stays blank and does not register on the map.
