Admin Reset After Failed Attempts

How are accounts unlocked after maximum number of login attempts reached? I have been testing this functionality (intentionally locking accounts), and have been unable to unlock afterwards.
Additional information – the test accounts are non-active directory accounts, and were setup only in WP.

I have just looked into the issue you described. I was able to recreate the problem.
At the moment, you need to explicitly exclude the usernames of local WordPress users from the Next ADI authentication. Go to the Next ADI plugin configuration page and enter the usernames you want to exclude by clicking on the User tab.
