Form and document plugins

Hi all- been tasked with creating a site where people can upload documents and complete a form (without logging in preferred but if needs be then they could as long as they cannot see other peoples submissions) and then having a document manager running where admin can see all submissions and form details.
I’ve tried a few things like CRM’s and basic document managers but I am really stuck as to which ones will work in this way! Can anyone advise me please? Many thanks
