Affiliate Application Accepted not generated

I have recently added the Coupon Affiliates plugin to my website. Initially, I have been using it to generate coupons but last week I created several new affiliate accounts with affiliate coupon codes.

I have configured a registration page and also activated the following options under Settings>emails:

New Affiliate Account Created
New Affiliate Application
Affiliate Application Accepted

Each of these three email types has been configured correctly, including the application accepted which is supposed to send an email containing shortcodes that allow the recipient to click straight into their account and add bank details, etc

However, when I manually add a new Affiliate account, none of the three emails get sent to the affiliate email address associated with their account. It doesn’t matter whether I manually enter the new account details from the WordPress dashboard or use the registration page to enter the details on their behalf.

Why would they not be receiving any of these emails? I tried to find somewhere in the settings where I can manually send them but there is no option.
