After click on button blank screen

Hi, i installed plugin, and added button on frontend, I created folder dkpdf inside put file….dkpdf-index.php

And i have blank screen…. of webpage…and url was changed with id….

And i have one more question, how can i do more templates for diferent custom post types ?
