Eli, I am having the same issues and I have run the malware several times. However the script can not be removed.

The files that could not be cleaned up where part of the super cashe plugin. deleted the plugin and ran the malware plugin again and the problem was fixed.

Yes, caching can make it hard to remove malware because it can continue to display infected pages even after you clean up the actual infection. Caching also writes a lot of files to your disk which can drastically increase the scan time. Always disable file caching and delete the cache files when dealing with malware.

P.S. After your site is clean and has been reviewed by Google (if necessary) then you can decide whether or not you really need to enable caching. I have found a lot of to perform better without caching plugins enabled. “Caching”, like “SEO”, is a very misunderstood and overly hyped up term, IMHO, and while some caching techniques can help your page speed if you get a lot of traffic, implementation is the key, and a lot of these popular caching plugins are not really speeding up most of the site I have seen them used on.
P.P.S. Measure your page speed at from a third-party site like http://gtmetrix.com/ and then turn off or on your caching and measure it again.

The names of the infected files vary with each infection. The important thing to look for is the patterns of the infected code within each file. My Anti-malware plugin does just that. If you have my plugin installed and updated and it is not finding this threat on your site then please post more details, the URL to the page you are seeing the infection on or a screenshot that shows your symptoms could be helpful.
