Thanks for posting! In taking a look at the form code on the right-hand side of your page, it actually looks like you may be using a different plugin than the MailChimp List Subscriber Form. For any questions related to that plugin, the best bet would be to reach out to the forum for that specific plugin (which will be available to access on that plugin’s page).
It’s possible you may have recently changed from our plugin to this other one. If you’re still looking to use our plugin, would you be able to provide me a link with the form in question that’s actively using our plugin? In doing so, I can take a look and make any suggestions to get you pointed in the right direction. Also, in your reply, can you clarify which screenshot you’re referring to?
