    I started to review the plugins code to see if I could propose a fix for the one time passwords not working correctly. As I got to the code that checked the one time password I found that there is a check for the plugin being active for the user. I remember seeing this option when I installed and configured it for my account but did not realize I left it unchecked. After activing this option the dialog no longer accepts an incorrect one time password by my testing.

    Did you first go to your profile and turn it on? Activating the plugin doesn’t do anything, you have to enable it for each individual user.
