Hi,
It’s not clear to me if you integrated the input fields in 2 places at the same time OR you tested it both ways (Divi and Contact Form 7).
Can you do a screenshot with the Form that you integrated it with?
I have used contact form 7 short code ([contact-form-7 id=”84″ title=”Book An Appointment”]) for this form.
