Avatar Upload Shortcode issue

I added the short code and it shows on the front page just fine. When I click the upload button after selecting my image, the page refresh and the image I selected is not there. I am using S2 Membership for my roles, and have giving the role the upload_files & edit_user capabilities. I am also only getting only the simple uploader to upload the image. Can you help me resolve this issue?
