Regarding your concern about not seeing the continue button on the billing address form. Please make sure and double-check that the correct key was used and if you use a Production or Sandbox Key, select also the same type of key in the dropdown button that matches it.

We can remove the Province from the Billing Address form. Just let us know if you wish to proceed with the customization and I will get back to you with the quotation.
