it looks like the page is served by an old version of cache.
If I visit for example https://ellasbubbles.com/walk-in-tubs/locations/chicago-il/?no-cache=sdfsdfdfsdfs I see the form without problems.
Clear the cache of Cloudflare, your server, and from your caching plugin, and check again the page.
Let me know if you still see the same issue.

Thanks for the response, but I am confident this is an issue:
This is because I have all plugins enabled (I would not leave the site broken)
See, if I simply disable 4 irrelevant plugins (even 1 will do it): https://i.imgur.com/aew17Z7.png
And then revisit that page, the individual page settings will override the front-end url and WPForms is down again… my only option is to enable ALL plugins if I want to override individual post types: https://i.imgur.com/j9oLG7O.png

It looks like the cached page is without form, and the non-cached page is ok.
If you are logged in you probably see https://ellasbubbles.com/walk-in-tubs/locations/chicago-il/ with the form because is not served by the cache, but if you visit the same page with a browser where you are not logged in, you don’t see the form.
With my browser I see https://ellasbubbles.com/walk-in-tubs/locations/chicago-il/ without the form and https://ellasbubbles.com/walk-in-tubs/locations/chicago-il/?no-cache=sdfsdfdfsdfs with the form.
I think https://ellasbubbles.com/walk-in-tubs/locations/chicago-il/?no-cache=sdfsdfdfsdfs is updated, and https://ellasbubbles.com/walk-in-tubs/locations/chicago-il/ is served by the cache. And the cache was generated before you disable the right plugins with Custom URLs => Frontend URLs.
I suggest you check the cache and check the page with a browser where you are not logged in.
If you are completely sure there are no issues with the cache we will investigate deeper. Honestly, I still think you have some issues with the cache.
