Calculated Fields Form does not load on website, cannot preview it

I have been using this plugin for years without any issues and a form that has displayed on the website for 3+ years is no longer displaying. I cannot preview it either. I tried reloading it via WordPress shortcode option and via the CFF block option. The form does not show up. When I click on the form url, I get a blank page. I cleared the cache, updated plugins and theme. and added /calculator/ to the Advance section of the urls to refrain from caching in WP Super Cache. Nothing is working and I am at a loss as to what to try next. Any suggestions?
