Calculation issue

    I want to add a ‘calculator’ on my wordpress site. Front end should look like this (link to img below).

    First 2 fields are dropdown menus (first 2 columns on the table). And then the submit should provide a number. The number is not technically a calculation, is just the result of the combination of the options from the dropdown menu options (third column in the table)
