    Hi,

    Yes, of course. You can create a form with multiple page, with only inserting a page break field between fields that belong to different pages, and in this case, with only insert the calculated fields in the last page, and configure the contact fields as required fields; the users can not move from a page to the other, if they have not entered the contact information.
