Calculator input fields

I am trying to set up a site for a courier service. The rate will be $13 plus an additional .75 per mile over 5 miles from the pickup. I need help to integrate the distance rate shipping plugin for woocommerce with a quote calculator for visitors so they can input their pick up location and dropoff location and plugin will calculate the cost and permit the visitor to purchase shipping if they accept the quote.
