Can Caldera Forms Do Multi-Select?

This form here: http://www.nahmforms.com/nahm-foot-form.php was created using php; but once in a while it stops working. I want a better solutions.
I found Caldera Forms; the dropdown menu seems like something I can use; however, what will happen when a customer wants the same item but in different sizes? For example, a customer wants T-shirt A in 1 size Small, 1 size Medium, etc. and the price will change accordingly. Can Caldera forms create something that?
