After adding the name, if hitting return does not work then please try to click on the button with the nose. See if that works or not and let me know.

Actually I meant to type Mouse but autocorrect replaced it with the nose.
Strange things happen sometimes. Anyway, click on the continue button rather than hitting enter and it will work.

You will see that the “Continue” button isn’t showing up. You’ll also notice that the list of forms is not like the grid it is supposed to be… I thought it had something to do with caching or Wordfence, but I deactivated them and still.. basically, the “Add New Form” page is broken. :\
