Cannot find a Multi-Path Questionnaire Plugin

    I would like to add a multi-path questionnaire to our site that provides different questions based on each answer given. I was hoping to find a plugin to do this, but have not had any success. I found many that allowed “one page only” questionnaires instead of multi-page.

    For example:

    1. Which kind of recipe would you like?
    a) Meat
    b) Vegetarian
    c) Desert

    If “a” then:
    Q. Do you like beef?

    If “yes” then: Here is a recipe for beef stew
    If “no” then Here is a chicken soup recipe

    If “b” then:
    …
