    I am not sure if this has always been a problem or not, but I was recently trying to insert uploaded images from my media library in a new website I am working on. When I click on insert into post from the media library screen it then links me to a not found page instead of inserting the pictures. I suppose I could code them in but that defeats the purpose of a cms now doesn’t it?

  • Have you tried:

    – deactivating all plugins to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s).

    – switching to the Twenty Eleven theme to rule out any theme-specific problems.

    resetting the plugins folder by FTP or PhpMyAdmin. Sometimes, an apparently inactive plugin can still cause problems.

    – re-uploading the wp-admin and wp-includes folders from a fresh download of WordPress.

    I have tried all the above trouble shooting steps still with no difference.

    I even tried completely deleting wordpress and re installing it. Still not working.

    I have now tried changing the permalink setting back to default I now receive a 412 error:

    Precondition Failed

    The precondition on the request for the URL /wp-admin/media-upload.php evaluated to false.

    Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

    That’s not a WordPress error. I think the server is borked. Try asking your hosts about that error message.
