you want to download this file:
http://geolite.maxmind.com/download/geoip/database/GeoLite2-City.tar.gz
from this page:
https://dev.maxmind.com/geoip/geoip2/geolite2/
After you unzip it (and untar it), you’ll find a file with extension mmdb in the corresponding folder. Rename it to maxmind.mmdb and upload it using your favorite FTP client to
/wp-content/uploads/wp-slimstat/
and you should be good to go. Please note that Slimstat will attempt to update that file once a month, when MaxMind releases their monthly update. You might need to repeat this process manually if you can’t find the reason why your server cannot download it automatically.

Im having exactly the same issue, and I have followed your above instructions 3 times, but it did not solve it.
The error I am getting is,

I have altered the web address to say just mysite.com as this is a forum.

It looks like your maxmind.mmdb is not a FILE, or if it is, its permissions don’t allow the web server to access it. Could you check with your favorite FTP client if that’s the case?

Do you have a direct support email to reach you on, I don’t always check the forums.
You are correct, however, every time I try to upload the maxmind.mmdb it simply creates an empty folder and not a database file?
Also, the heatmap doesn’t not seem to be working.
I have followed all instructions on this, and can not seem to get either issue to work.

We are investigating this issue. In the meanwhile, you can simply delete the empty “maxmind.mmdb” folder as a workaround to fix this problem.

Do you have any suggestions for the heatmap as well? I can turn the heatmap on but there seems to be no history of movement being saved and displayed.

Please make sure you’re using the latest version. If you are, please contact our support team at http://support.wp-slimstat.com so that I can send you a copy of the next version of Slimstat, which should fix some issues with the Heatmap add-on.
