Thanks for reaching out! I’m sorry to hear you’re running into issues with the Form Builder on your site.
Looking at the screenshot with the console errors, these types of issues are often caused by a conflict with another plugin or your theme. I saw that you mentioned testing with a default theme and deactivating all plugins. However, I recommend the following:
- Uninstall WPForms Lite completely, then reinstall it. The issue might have occurred during the update from the old version to WPForms Lite 1.9.7.1.
- If the issue persists, please try testing for a conflict again using our guide here.
Also, please confirm you’re not using any JavaScript optiizations on Cloudflare, such as Rocket Loader.
Once you’ve had a chance to reinstall WPForms Lite and try the steps above, let me know how it goes and if the issue is resolved.

I reinstalled the plugin, no chnage.
Cloudflare Rocket Loader is off.
Default WP theme and disabled all plugin, no change.
New form creation works though. The form was created in 2018 latest plugins, probablz in the last 7 years of update corrupted the form backend. The actual form on the front end works.

I agree, it’s likely that the issue with your old forms is due to some kind of corruption. Fixing that would require reviewing the form data in the wp_posts table.
That said, if recreating the form works for you, that’s definitely the easiest solution. I’m happy to hear that the current form on your site is working as expected.
