I click activate, the message status say’s Plugin Activated, however, I scroll down and the plugin is actually not activated.

That sounds like a problem with some sort of caching. Check (and flush) any cache you can find. Also check with your webhost to see if there is some sort of PHP cache (like APC) as well.
