Can't Authenticate Without Refresh

    I have about 20% of my users having this issue. It happens on all the browsers / OS combos.

    When they try to login, they get this error:

    Simple LDAP Login: Simple LDAP Login could not authenticate your credentials. The security settings do not permit trying the WordPress user database as a fallback.

    If they refresh their browser and try, they are able to log in.

    Anyone have any ideas?
