This problem occurs if these forms are created with a very old version of Form Maker (1.6.x or lower). Was this your case?
If so, unfortunately, you need to recreate these forms 🙁 But, recreating them will let you use the newest features of Form Maker.
But in case the forms were created with a new version, please contact our team using this form (select “I have a pre-sales question” and the form will appear).

I don’t know which version of FormMaker we used, but the newest form is dated May 19, 2014, and we have not needed to update any of them for 3-1/2 years.
I went directly to the WordPress MySQL database and found FormMaker tables that reveal a lot of details about each form. But one piece of data I can’t find is the email address where a form is sent after submittal. Where would I find that?

Please note, that the version of May 2014 is a very old version. So unfortunately, you need to recreate your non-working forms.
As for your second question, the recipient email addresses are under mail column of [prefix]_formmaker table. You can also set it from Form Options > Email Options page, using Email to send submissions to option.

I am also having this same issues – brand new install of this plugin. When you create a form, publish and then go back to edit its does not work.
After inspecting the code it is riddled with error and console errors.
Please can you take a look – urgent request.
