can't see the button

The button doesn’t show on my toolbar.

I’ve tested on WP 4.2 to 4.3.1. The only, current, thing I’ve not tested is today’s release of TinyMCE 4.2.5.
What versions are you seeing this on?
FWIW Just updated TinyMCE to 4.2.5 and the button is still there.
I use Black Studio TinyMCE Widget 2.2.8

Sorry, I have the standard WP build, plus the “TinyMCE Advanced” module ( https://ww.wp.xz.cn/plugins/tinymce-advanced/ ). The screenshots on the Black Studio one show a different toolbar. It doesn’t show the “undo / redo” buttons which the button normally appears next to.
If I get a chance at the weekend, I’ll try and find a way to test with that module.
No problem, I can install your TinyMCE and solve the problem in this way!

Tested on a clean site today, versions:
- WP 4.3.1
- Z-URL Preview 1.4.4 (although nothing about MCE has changed since release)
- Black Studio TinyMCE Widget 2.2.8
The button showed up on the toolbar.
Don’t know. It was easier installing another tinymce plugin.

button does not show. I have tried deactivating and reactivating, deinstalling and reinstalling, absolutely nothing!

update, am I looking in the wrong place? The button appears when I am editing a page, but not when I am editing a blog post. Is this the correct behaviour?

update2, click on the button, enter a URL, click OK and nothing happens on the page.

Those versions seem to match with what I tested and screenshot(ed). But will double check over weekend.
What specific URL fails? As some sites do not parse well π Try a “standard” news.bbc.co.uk news article to see if it is a problem with the target URL/site or the plug-in.
If some URL’s/sites fail, but others work, then it’s a parsing issue. In which case I will need a specific URL to attempt to fix it for that site.
Some sites will never work, but specific URL’s will let me try and fix the problem for main-stream sites.
Those are the versions I tested on and I see the button on posts and pages.
Given your followups where you do see the button, I can only guess it’s a problem with the source URL.
