If I have the default/php (or any) of the send options checked, I can’t even send the test email. The button is inactive and says, “You cannot send an email. Mailer is not properly configured. Please check your settings.” Do you know why this would be, even if just the default setting is checked? Is it a server issue?

No, this a default behavior. There’s little reason to have plugin activated and still using Default mailer. Thus we block test email for default mailer.
Plugin fixes email deliverability only when something else than Default is selected and configured.

Anyway, we will unblock test email for default mailer in 1.4.1 coming next week.
