Try logging out of WordPress and then refresh the form page to see if the fields are displayed.

Then I’m not sure what the issue could be.. normally if you enable the settings:
– Registered Username
– User Profile URL
Then the Name and URL fields are hidden when the user is logged in. When the user is logged out, the fields are displayed. But if you say that you’ve tried logging out and they are still not showing, then most likely something like another plugin or your theme is interfering with normal functionality. I recommend some basic troubleshooting to determine the cause of the issue.
maybe is because i am using “ultimate member” plugin and people are logged in with their users accounts, and with the new plugin update could create a conflict with this plugin. If people is logged in with their user name it is absurd to put again an user name.

Ah yeah that would explain it. If the user is logged in and those settings are enabled, their name and URL are grabbed automatically from their User Profile, so there is no need to include the fields on the form. The information is sent automatically behind the scenes in that case.
