I am sorry to read about your issue. However, we will need more information to be able to understand the context.
Did you just install the plugin and you didn’t manage to stop the spam, or is this an upgrade and all of a sudden you are getting a lot of spam?
Also, what version of CAPTCHA are you using?
Can you see the CAPTCHA check on the comments form?
The more information you can give us in regards to the setup, the better we can understand the issue.

Therefore it seems like the plugin is working well. There will always be the occasional comment etc – CAPTCHA is a bit like your antispam mailbox; while it blocks the majority of spam, a few might still make it through.
