I hope you’re doing well and I’m sorry to see that you’re facing this issue.
That doesn’t seem to come from Forminator, as usually it doesn’t show the Captcha like that, but could you confirm if deactivating Forminator helps?
In case it does, could you please try to create a backup and reinstall it by uploading the zip under plugin > Add New > Upload Files and replacing files?
In case deactivating Forminator doesn’t help, could you give it a try with a malware scanner like the one on Defender? https://ww.wp.xz.cn/plugins/defender-security/
You could need to reinstall the WP core if you find any malicious files there, but remember to create a backup before taking any action.

Hello, tried to disable forminator and the captcha does appear. Tried also to install defender and no malware detected. I don’t have any other plugin but Duplicator and Forminator and really I don’t know where to look at.

Maybe the browser extension, device, or website has malware causing the issue. Have you tried visiting the same page using a different device or mobile browser? Are you replicating the same issue?

Hello, solved the problem. The site was infected by a malware in the plugin directory. I’ve checked the source code and found a script encoded, searched on the server and there was an HSEO folder inside plugin that’s not related to anything. Removed that folder and the problem is gone.
Quite strange that the plugin Defender doesn’t catch the malware though.
