I cannot duplicate the problem on my test servers. Please provide a URL to your form so I can try to duplicate the problem.

I was able to attach a test.txt to your form with no problem.
What type and size files are you testing with?

Hmm… I tried on a different browser — one where I wasn’t logged in to WP — and it worked. I wonder if it has something to do with caching? In any case, it appears that as long as someone isn’t logged in, it works fine. (I was using a <1MB JPEG the whole time.)

Yes I think it is a cache issue. Try to adjust your cache plugin to ignore your contact page. Also when you upgrade the plugin to version 4, the captcha does not use php sessions and this might not happen

That did the trick! I added the keywords from the URL string to my SuperCache list and it works now, even when I’m logged in. Thanks!
