Captcha Not Showing

I had the captcha showing up on fine on my form on my webpage on justrightair.com. This last week, the captcha doesn’t show up. Here is what I have done to try and fix it, but to no avail.
* Made my upload folder read/write 777 to all subdirectories
* Made sure GD and freestyle are installed on our server
* Upgraded our Cpanel version to latest version

I do have Really Simple Captcha installed
