Change form layout into two columns

    Hello,

    I am trying to change the upload form layout to a two column form.

    Left column fields:
    Email address
    Name
    Company name
    Phone number

    Right column fields:
    Title
    Post content or file description
    Your media files (file upload)
    Submit button

    Can you please share if and how this is possible?
