Change Password Fields Missing

    All plugins disabled, using WordPress’ default theme. Change password fields are still missing.

    This is a self-hosted WordPress blog, and it’s on a dedicated virtual server at MediaTemple. I didn’t install via Plesk, it was a self-install.

    I realize I could go into phpMyAdmin to change the password, but I’d like those fields back. I don’t like the idea of not being able to easily change my password when the need to do so arises.

    Suggestions?
