For this website I want to make a pop-up for collecting email addresses. This site is build in Divi and use the MC4WP: Mailchimp for WordPress version 4.5.5 plugin.
All fields show up but not the checkbox or radio. How can I fix this?

The pop-up is currently offline due to this issue

Hi,

The checkbox is within the <label> … </label> tag, and your theme is hiding all labels.

In the form editor (MailChimp for WP > Form > Fields), just delete the <label> and </label> from around the checkbox code.
