Checkboxes only recording one answer

The jQuery script which manipulates the form and fixes the checkbox problem (and a number of other things) is missing from the page source. It should be output toward the bottom of the page as part of the wp_footer action.

Something (theme, plugin, ?) has either unhooked wp_footer actions or is preventing it from running.
