Checkout form customization does not work

I’m helping with Saloon reservation plugin setup for a website.
We need a checkbox for “I agree with Terms and Conditions” in the checkout form.
1. I’ve added the field in the backend
2. It does not show in the Front-end (does not exist in the HTML – if you inspect)
3. An error is displayed if the checkbox is set as requiredThe WordPress version is 5.8.1
The plugin was 6.x.x, but I updated it to 7.1. and still same problemp.s. tried other field type – same issue – does not show

The Checkbox is added in the Admin with a label and it is Required. The validation fires and ask the checkbox to be “checked”
Still in the Frontend, the checkbox is not visualized at all – only the default fields.
