CoBlocks Form Not Submitting

The first attempt to submit the form fails. No error message. Screen refreshes to blank form and nothing has been submitted. The next attempt will work.

This is how it goes; works one time, then not the next, then works the next time, then not the subsequent time, etc. Submits – Doesn’t Submit- Submits- Doesn’t Submit- Submits etc

I have tried browsers Chrome, Microsoft Edge, Safari, same thing happens on all.
