Code generator not showing up

WP 4.5
WooCommerce 2.5.5
Debug enabled

When clicking on the button I get this error in the console:
GET https://torinochokolade.dk/wp-admin/admin-ajax.php?action=woocommerce_produ…l_all_in_one_shortcode_generator&width=750&height=409&random=1461651537597 500 (Internal Server Error)

I made a test using the example shortcode, I get this error when I look at the preview in Divi:

Undefined variable: index in /home/torinoch/public_html/wp-content/plugins/woocommerce-products-carousel-all-in-one/carousel-generator.class.php on line 247
