Could you let me know what other plugins are currently active on your site?
Akismet, Yoast SEO and Analytics, All in One WP Security, Envira gallery lite,
One click close comments, Visual form builder, Ultimate category excluder
Could you try to deactivate All in One WP Security, and One click close comments, and let me know if it helps?

I’m having an issue with comments on my WordPress website.
User fills out the form and I receive notification of new comment in WordPress, but when I go to check it out there is no username, email or comment content.
What could cause this? I’m not using captcha plugins (I’ve seen this to be a problem before)
