    Dear emtenzer,

    Could you please tell us if you have set the questions as field labels? If yes, then Form Maker does send them with the submissions. But if you have set them as Custom HTML fields, please note, that you need to add it to Form Options > Email Options > E-mail to admin/Email to user section’s editor.
