Complex Form – Plugin or Move Across?

    Hi,

    I’m looking to implement the form on this page:http://thedentalimplantclinic.co.uk/referral.htm onto the new version of the site, based on WordPress.

    As you can see, it’s quite a complicated form, and the plugin I’m using at the moment (Contact Form 7) doesn’t seem to be able to create anything like it. What’s the easiest way I can replicate the form on WordPress? Is there a plugin I can use, or should I try to copy it across and submit it some other way? If the latter, how would I go about that (I’m not experienced with PHP, or WordPress for that matter, and the old form submits with an ASP script).
