Conditional doesn’t work for the 2nd checkbox

    Hi! I’ve copied the link of the test result of my form.

    I want to show an embeded form from another platform (zoho) when a checkbox is selected. It works only with the first checkbox and not for the second. I tried to swap it and it works always for the first one only.

    Please, can you give me some advice to fix it? Thanks!

    I have no experience with this, but it might have something to do with the fact that you are loading the exact same script 2 times.
