conditional fields now displaying on form

I’ve been using the conditional fields for contact form 7 plugin for some time on a website and recently, it has stopped working and is not showing the conditional field groups.
I have done all the usual basic checks and updated WordPress to the latest version, updated all plugins, checked the browser console for javascript errors but it is still not working. Could you please advise as to how to fix this.

The form code and settings of the conditional fields screen, haven’t changed since the form was working. I wonder if you can be of any help to see why it has stopped working.
